It was an arduous, time-consuming effort! There is lot of info, but issue with most of the info is that you need to spend days reading and understanding it before you can start! I organized it in such a manner that you can just start off, and do it reading one step at a time
steps-to-apply-oci
1. take photo in costco. get 2 prints and digital copy. they will give in usb stick. tell them it is for india. doing in costco is good, because, for example, they gave me tip that if my son wore white shirt, photo may be rejected.
2. check kids passport signature. get similar signature in white paper and scan. then you have to convert to aspect ratio 1:3, 200x600 pixel size. I used "Irfan view". It is free.
3. get copies of following docs scanned. each item must be a single file
- first page of pge bill with address
- OCI of kid
- passport of kid: photo page, signature page, page 24,25,26,27 endoresemets even if blank
- birth certificate of kid
- passport of both parents: same pages as above
- marriage certificate of parents
- parents OCI card
- THIS MAY NOT BE NEEDED. indian passport showing "cancelled because attained US citizenship". 2 pages with photo in front, and 2 pages at end
5. keep following handy
- oci of parents and siblings
- address of one relative in India
5. start filling application in https://ociservices.gov.in/welcome -> OCI misc services -> make sure you select
- change of passport particulars
- change of address
- enter fathers, mothers and siblings OCI details in question 1 in part B. remember to enter full name as in passport and relationship
- enter one relatives detail in corresponding uestion in part B
Write down the file number created. Starts with USAS***
6. (DO NOT PRESS "FINAL SUBMIT" at this step). Upload docs in the last step of applcation in govt site, BUT DO NOT PRESS "FINAL SUBMIT". Some of the uploads are clear, some are confusing. I did this:
- address proof - PGE bill
- current passport - kids current passport
- uploaded kids OCI as India visa
- uploaded kids birth certificate as "Relationship certificate"
- uploaded passport of both parents as "LEGAL STATUS OF PARENTS (IN CASE OF MINOR APPLICANTS)"
- uploaded parents marriage certificate as "marriage certificate"
- THIS MAY NOT BE NEEDED. i uploaded it anyway. uploaded parents indian passport showing "cancelled because attained US citizenship" as "Renunciation certificate of indian citizenship"
- uploaded parents OCI card as Indian origin proof
7. go back to https://ociservices.gov.in/welcome. click "reprint submitted apps" -> OCI misc. print as pdf and verify. both parents sign page 3
8. go to ckgs website: https://www.in.ckgs.us/oci/
- enter all details. only confusion is that you have to select "At CKGS center" as mode of payment
9. at next step, the CKGS website will give order form. save it. then click to proceed on the button which says so on top
10. at next step, the CKGS website will give affidavit in lieu of originals form. fill everything. tick address change if there is a change in address.
11. go to my account in CKGS website. it will list a bunch of docs. print "parental authorization form"
12. take the 2 affidavits above and page 3 of application to notary. get notarized. only thing you need to take are IDs for parents and kids. we took kids passports, parents license.
you need to notoarize 2 parents signature for each form - so 6 notarizations in total. I also notarized 1 kid signature for affidavit in lieu of originals - this may not be needed
13. scan the notarized docs.
14. go to https://ociservices.gov.in/welcome -> reupload docs. upload notarized "parental authorization affidavit"
15. press final submit. save application form
16. print following (make sure nothings gets cropped, use fit to size in printe dialogue)
- application form pages 1-3 (print again, because first page changes after submitting new doc)
- address proof (pge)
- kids passport
- parents passport
- parents oci
- parents cancelled india passport (THIS MAY NOT BE NEEDED)
- kids OCI
- kids birth certificate
- parents marriage certificate
17. arrange in following order
- application form pages 1-3, notarizations
- address proof (pge)
- kids passport
- parents passport
- parents oci
- parents cancelled india passport (THIS MAY NOT BE NEEDED)
- affidavit parental authorization, notarizations
- kids OCI
- kids birth certificate
- affidavit in lieu of originals, notarizations

18. go to ckgs website
- there is a "print barcode" dialogue (to the right of list of docs). print
- there is a "print label" dialogure (to the right of print barcode dialogue). print
19. go to fedex.
